Insurance

As a courtesy, we will file claims with most any insurance company for our patients as long as we are supplied with the appropriate information. The bill is your responsibility whether the insurance company pays or not.

An insurance policy is a contract between a patient and the insurance company. Therefore, if insurance does not pay in a timely manner, it is your responsibility as a patient to follow up with them.
 
At Performance Injury Care & Sports Medicine in Helena, Montana, we are contracted with many insurance companies. Most of the services we provide will be processed in network with these companies. Some of the major insurance companies we are contracted with include:

Health Rehabilition Helena Insurance Providers

If you do not see your insurance company listed here, please call and ask our billing department about it. We also encourage you to call your insurance company and verify eligibility and coverage.

There are several Insurance Companies we do not contract with. We will bill these companies on your behalf, but these will more than likely process with out-of-network rates. You will be responsible for the balance. The major companies we do not contract with are:

  • Humana

  • Aetna

  • Mountain Health Co-op

The companies we can no longer bill on your behalf are:

  • Montana Medicaid DPHHS

  • Healthy Montana Kids

  • Other State Medicaid

  • Federal Workers Compensation

Our providers are committed to delivering the best treatment possible for our patients and charge what they feel is usual and customary for our area. The patient is responsible for payment in full regardless of any insurance company’s determination of usual, customary, or allowed amounts.

Many insurance companies pay our fees in full. If you feel your insurance company has paid incorrectly, please request a review of your claim. By following up with your insurance company, you will ensure that you are being reimbursed to the fullest extent of your coverage.

Insurance / Payment / Billing

At Performance Injury Care & Sports Medicine, we accept workers’ compensation, and Medicare, and are contracted with most major insurance plans. Please contact your insurance company or you may call our office at (406) 422-5817 to find out if we accept your plan.

If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience, we accept cash, checks, Visa, MasterCard, Discover, and American Express.
